After consulting with OCE on potentially using Qualtrics for our tester signup form (see #297 ), we learned we can use Touchpoint for the form. We need to copy what we have in the Google forms signup into Touchpoints

I just created the signup form in Touchpoints (mostly copied what was in our Google form). Take a look at the preview and share your thoughts @amycole501 @annepetersen

Here are some suggested changes, but I defer to @annepetersen on the final copy.

US Web Design Service Usability Testing: Participation Signup Form

The U.S. Web Design System (USWDS) is conducting usability studies to better understand the needs of people with disabilities and/or people who use assistive technology.

These usability sessions typically take an hour. They are conducted via a webinar or remote meeting platform.

You will be asked to interact with a website and share your thoughts and complete tasks. Our goal is to discover usability and accessibility issues with our site.

You will be compensated for your time.

If you are interested in participating, please leave your email address in the form below. We will contact you when opportunities are available.

  • In the first field just ask for email address
  • In the name fields just put First Name and Last Name. Those are all common conventions people will be used to.
  • For the organization field, do we want that one required or optional?

Thanks! Yes I have been going back and forth on the last one being required or optional. It's the only way I have some idea of what type of disability the person filling out the form might have. But if someone is just coming to our site (without being referred by an org) and signs up, this question might not make sense to them...

I incorporated your feedback and edited the last question so that it allows people to answer "N/A" or 'inapplicable' in case no organization referred them. It's still required which I feel is probably the way to go, but open to discussion there:

Ok I've made the last question optional and also edited the introductory paragraphs to the survey based on Anne's feedback. I think after a content strategist weighs in, we can wrap this up. @sarah-sch or @finekatie can you please take a look at the survey preview and let me know if you suggest any changes, particularly on the following copy:

Here at the U.S. Web Design System (USWDS), we’re hoping to talk with people who are disabled and/or who use assistive technology so we can better support your use of government websites.

We do this remotely through a video call, which usually takes an hour. We’ll ask you to share your thoughts, mostly while using a website we’ll provide. You’ll be compensated for the time you spend with us.

If you’re interested, please let us know by providing the information below, and then we can email when we have an appropriate opportunity for you.

Sarah has reviewed this survey copy from a CS point of view. I've updated the survey (preview here) and it is ready for final review and approval @annepetersen and @thisisdano
