Documentation-specific issue and pull request template

Expected Behaviour

It would be really helpful if there were standard ISSUE_TEMPLATE.md and PULL_REQUEST_TEMPLATE.md files which were useful for documentation-only projects.

Actual Behaviour

These don't exist :(

FWIW, I took a stab here:

The pull request one was simple since it is just a reduction of the normal one. I honestly don't have enough experience with docs issues to figure out what would make a good template.
