Looks like squid also needs to allow access to .eessi-infra.org. At least I couldn't get cvmfs to work without this:

acl stratum_ones dstdomain .eessi-hpc.org .eessi-infra.org

Thanks! We recently changed the URL, so this definitely needs to be reflected in the example template file.
